Payment and Insurance Reimbursement
We do accept most insurances. Participants are responsible for co-payments, deductibles and payment for any services that are not covered by insurance.
- We accept Beacon Commercial, Non-Commercial, HMO’s, Non-HMO’s, NYS Empire Plan, CDPHP, Cigna, Emblem Health, Fidelis, GHI, MVP, NY-HMO-CHCS, Optum Plans-United Behavioral Health and Value Options.
Our standard fees are: $150.00 for the Initial Evaluation, $120.00 for Individual Therapy Sessions, $150.00 for Couples Counseling and Family Therapy sessions. Fees are based on 55-60 minute sessions. Infant and Preschooler Mental Health services are provided as Individual and/or Family services.
Groups are available based on service need and costs are based on class size, materials and/or books used. The average group is $50.00 per person and is covered by most insurance companies. Crystal Sun services may offer groups at a lesser rate based on larger class sizes and lower costs for materials.
Payment for all services is due at the time of service.
We also accept checks, cash, major credit/ debit cards and some health passports. Clients who carry insurance should remember that professional services are rendered and charged to the client and not to the insurance company. If your insurance is accepted and billed, there may be a co-payment or deductible set by the Insurance and the client is responsible for this.
